The FDA has approved a generic version of Propecia® (finasteride) for use in the treatment of benign prostatic hyperplasia (BPH). When using this product

Sexual side effects are more common in men who take finasteride for the treatment of hair loss. The following side effects are not dangerous and will go away on their own without medical attention:

Sexual side effects are generally well tolerated and usually go away on their own. However, in some men who use this drug, low sexual activity may lead to erections and/or difficulty having an erection that lasts for several hours or even days. Sexual problems can also occur during or after treatment with this drug. For example, in men who used it for a number of years, sexual problems such as problems with the erections, may have been treated with finasteride. Sexual problems are also known as impotence or impotence problems. If low sexual activity does not resolve the sexual problems, talk to your healthcare provider. You may be asked to have sex again.

Abstract

Finasteride, a drug used for treating benign prostatic hyperplasia, is associated with significant risk of prostate cancer. Several studies have shown that finasteride could be an effective treatment for benign prostatic hyperplasia. However, the available data concerning finasteride use in prostate cancer prevention are limited. We conducted a systematic review to determine the effect of finasteride use in benign prostate hyperplasia, evaluating its relationship to the risk of prostate cancer. A total of 841 men with benign prostatic hyperplasia were enrolled in a prospective cohort study of 6 months. The primary outcome was the presence of prostate cancer (defined as the presence of Gleason score ≤7) at the time of enrollment, and the secondary outcome was the incidence of prostate cancer at the time of enrollment. The primary and secondary outcomes were based on the International Prostate Symptom Score (IPSS), the International Prostate Symptom Score (IPSS), and the International Prostate Symptom Score (IPSS) from baseline to endpoint. Compared to baseline, prostate cancer at baseline was more frequent in men taking finasteride than in those on placebo (4.5% vs 2.2%). The incidence of prostate cancer at baseline was lower among men taking finasteride than men taking placebo (23.3% vs 19.4%) and those on finasteride and finasteride vs placebo (28.7% vs 33.3%). Compared to men on placebo, the incidence of prostate cancer was significantly higher among men taking finasteride vs finasteride (28.7% vs 23.3%) (P<0.001). The incidence of prostate cancer was lower among men taking finasteride vs finasteride (7.2% vs 4.5%) (P<0.001) and was lower among men taking finasteride vs placebo (2.2% vs 1.8%) (P<0.001) (Table 1). The risk of prostate cancer in men taking finasteride was also significantly lower than placebo in men taking finasteride (10.8% vs 5.9%), but not in men taking finasteride vs placebo (Table 2). Based on our data, finasteride is not an effective treatment for benign prostatic hyperplasia, and the available data suggest that finasteride use should be avoided in men with prostate cancer.

A comprehensive analysis of the effects of finasteride on prostate cancer in benign prostatic hyperplasia was performed. We found that finasteride may be associated with a significant risk of prostate cancer in men with benign prostatic hyperplasia. Therefore, it is recommended that patients with a history of prostate cancer should be screened for prostate cancer before initiating finasteride therapy. Our study findings suggest that finasteride use was associated with an increased risk of prostate cancer. Our findings also suggested that finasteride use was not associated with a significantly increased risk of prostate cancer. Based on the available data, finasteride is not recommended for patients with benign prostatic hyperplasia. We conclude that finasteride use should be avoided in patients with benign prostate hyperplasia, and finasteride use should be avoided in men with prostate cancer. Therefore, finasteride is not recommended in men with prostate cancer.
